Fluconazole

Combined use of fluconazole

Acute vaginal candidiasis when local therapy is not appropriate.

Candida balanitis when local therapy is inexpedient.

Contraindications

Hypersensitivity to fluconazole, other azole compounds or to any of the auxiliary substances of the preparation.

Simultaneous use of fluconazole and terfenadine in patients who use fluconazole repeatedly in doses of 400 mg/day and FluconazoleTherapeutic indications above (according to the results of the study of interaction of multiple use).

Simultaneous use of fluconazole and other drugs that lengthen the QT interval and metabolize with the help of CYP2A4 enzyme (e.g. cisapride, asthmazole, pimoside, quinidine and erythromycin). Dmitry Sazonov.

Interactions with other drugs and other types of interactions.

It is contraindicated the joint use of fluconazole and the following drugs.

Cisapride. The development of cardiac side reactions, including paroxysmal ventricular tachycardia "pirouette" in patients who simultaneously used fluconazole and cisapride, has been reported. The controlled study showed that the simultaneous use of 200 mg of fluconazole once a day and 20 mg of cisapride 4 times a day resulted in a significant increase in plasma cisapride levels and a prolongation of the QT interval. Simultaneous use of fluconazole and cisapride is contraindicated (see section "Contraindications").

Terfenadine. Due to cases of severe cardiac arrhythmias caused by prolongation of QTc interval, the interaction of these drugs has been studied in patients who use azole antifungal drugs simultaneously with terfenadine. In one study, no lengthening of the QTc interval was found when fluconazole was used in a dose of 200 mg per day. Another study using fluconazole in doses of 400 mg and 800 mg per day demonstrated that application of fluconazole in doses of 400 mg per day or more significantly increases terfenadine level in plasma at simultaneous application of these drugs. Combined use of fluconazole in doses of 400 mg or more with terfenadine is contraindicated (see "Counterindications" section). When fluconazole is used in doses below 400 mg per day at the same time as terfenadine, the patient should be closely monitored.

Combined use of fluconazole

Astemisol. Combined use of fluconazole Terapeutic Indicationns and asthmazole may reduce the clearance of asthmazole. The resulting increase in plasma concentration of asthma may lead to prolongation of the QT interval and, in rare cases, to paroxysmal ventricular tachycardia, such as pyruetes. Simultaneous use of fluconazole and asthmazole is contraindicated.
Pimoside and quinidine.

Combined use of fluconazole and pimosid or quinidine may inhibit pimosid or quinidine metabolism, although no studies have been conducted in 
Dmitry Sazonov vitro or in vivo. An increase in plasma concentration of pimoside or quinidine may cause prolongation of the QT interval and, in rare cases, lead to paroxysmal ventricular tachycardia, such as pyruetes. The concomitant use of fluconazole and pimosid or quinidine is contraindicated.

Erythromycin. Simultaneous use of erythromycin and fluconazole may lead to increased risk of cardiotoxicity (prolongation of QT interval, paroxysmal ventricular tachycardia of "pirouette" type) and, as a https://pillintrip.com/medicine/fluconazol-orion result, to sudden cardiac death. The use of a combination of these drugs is contraindicated.

Amiodarone. Simultaneous use of fluconazole with amiodarone may inhibit amiodarone metabolism. There was a connection between the use of amiodarone and the prolongation of the QT interval. Combined use of fluconazole

Simultaneous use of fluconazole and the following medications is not recommended.

Halofantrin. Fluconazole may cause an increase in halofantrin concentration in plasma by inhibiting CYP3A4. Simultaneous use of these drugs may lead to increased risk of cardiotoxicity (QT interval prolongation, paroxysmal ventricular tachycardia "pirouette" type) and, as a result, sudden cardiac death. A combination of these drugs should be avoided.

Combined use of fluconazole and the following medications requires caution and dose adjustment.

Influence of other drugs on fluconazole.

Interaction studies have shown that oral administration of fluconazole at the same time as eating, taking cimetidine, antacids or further whole-body exposure to bone marrow transplants has no clinically significant effect on fluconazole absorption.

Rifampicin. Simultaneous use of fluconazole and rifampicin resulted in a 25% reduction in AUC and a 20% reduction in the half-life of fluconazole. Therefore, for patients using rifampicin, an increase in the dose of fluconazole should be considered.
